Claim Decision, but moved back to step 4 with nothing deferred?

Hi Everyone,

Looking for some similar stories as I can't find anything that matches my situation online. I recently submitted a claim for a couple of conditions, and received my decision letter online a few days ago, and they were both service-connected and rated (overall rating stayed the same though). However, my claim didn't close and instead went back to step 4: evidence gathering. Everything I've seen online says this is common for deferred items, but I don't have any deferred conditions, so I'm confused. Called the VA and they didn't know either. Anyone have anything similar happen? What was the outcome?
