Who do I report unsafe housing conditions to?

The apartment that my family and I have been at since we came to America (2007) has had constant problems from mice to ants and now roaches. It’s getting unbearable and I feel like I need to report the landlord so something can be done. We are clean people. Multiple families and both buildings are experiencing the same things. I’m moving out soon but I don’t want to leave my family in these conditions. I looked at the mn department of health site but there’s no contact information. Who do I contact about this?
