Fish keep dying after adding water. Why?

I’ve had guppies since august and they do great until it’s time to top off their water. We use the Top Fin preconditioned water when the tank water starts to evaporate. A couple of days after we top it off one fish will get sick (bloated stomach, trouble swimming) and then die. What are we doing wrong?
