Fissure treatment

Hi, I (23M) have been dealing with bad fissures after a surgery and need some advice on whether I should try Botox or get LIS surgery. I've tried every ointment and seems I will have to go the Botox or LIS route. Any advice from anyone who has had one or both of these procedures would be greatly appreciated.

As a bottom, I'm really fearful of my future sex life after this situation.

For context, I've been pretty rough on my hole in the past and never got fissures, and also never had a hard time relaxing my hole for a partner (I've even been called loose in the past).

I'm worried Botox won't be enough, will retear in the future, or need LIS anyways. Every day has been a living hell due to the pain and I want this fixed.
But with LIS, I'm worried about incontinence and being permanently looser, especially as I get older. Bespoke Surgical seems to heavily recommend against it for those reasons, but I am not 100% trusting of one article from one private practice.
