Why did I get the “failed as a mentor” achievement for Visi?

I’m not the best at these kinds of games so it isn’t much of a surprise, but I’m also confused. I chose to support Invisigal in every opportunity I got (or at least I think I did). I went to the movies with her, untied her, and forgave her with every option regarding Shroud. The only thing I failed with was a few of the hacking parts when Vis tried to get the astral pulse, and I kissed Blonde Blazer in the first episode. Does that affect the ending or did I make some other mistake?
