First time submitting to a poetry contest. Help please !

Hey y'all

Looking to finally make a submission for my first poetry contest . I have the poem I want to submit but where I'm having a problem is writing the bio for my cover letter .

Anyone have any suggestions or templates that they use ?

Thanks
