Personnel Complaint Form for APD

Hey everyone,

I got pulled over last night for a number that peeled off the back license plate (it wasn’t even my car, I was driving my parents car to grab some culver’s lol) and the officer that pulled me over started pressing me about some stuff that made me very uncomfortable and stuff that was NOT true.

I am trying to find the Personnel Complaint Form on the APD’s website and I can not find it anywhere…. Has anyone had to do this before and where did you find the form? If anyone has had to do this before, what was your experience with how it was handled?
