Dilaudid/hydromorphone dosage

What does is everyone on? My doctor is going so slow with the increases. I'm only at 1.025mg a day. No relief so far. The nurse practitioner does the increases every week. But next month I see the actual doctor to see why I'm not getting relief. He thinks I should be getting relief by now.
