Bloodwork / Next Steps

NSFWQuestion

22M, 165lbs

Hi,

I’m looking into TRT despite being young. I need help - am I delusional or could TRT be an answer for me? I feel quite lethargic quite often, have brain fog, low libido, slightly depressed and have a hard time gaining muscle and losing fat despite a very disciplined diet and training regiment. I don’t drink or smoke and essentially just live a healthy lifestyle, trying to stay active when able.

I have gone to my PCP and an endocrinologist and they essentially dismiss me due to my bloodwork, though my symptoms are all leading towards a hormonal issue. I will link my bloodwork below. I am “in range” for most, but my free androgen index is low. I also used to sit at ~800-900ng/dL for total test but that has dropped to ~500 in the past couple of years.

• Testosterone-to-Estradiol Ratio (T/E₂): 1.71 — Out of optimal range (low)

•	DHEA-Sulfate (DHEA-S): 102 ug/dL — Normal (below optimal range)

•	Sex Hormone Binding Globulin (SHBG): 42 nmol/L — Normal (slightly above optimal range)

•	Estradiol: 30 pg/mL — Optimal / In range

•	Free Testosterone: 57.1 pg/mL — Normal (below optimal range)

•	Total Testosterone: 513 ng/dL — Normal (below optimal range)

•	Bioavailable Testosterone: 112.5 ng/dL — Normal (below optimal range)

•	Free Androgen Index: 42.38% — Out of range (low)

•	T4 (Thyroxine), Total: 5.4 ug/dL — Normal (below optimal range)

•	Free Thyroxine Index: 1.7 — Normal (below optimal range)

•	T3 Uptake: 32% — Optimal / In range

•	Thyroid-Stimulating Hormone (TSH): 3.14 IU/L — Normal (above optimal range)\*\*

I feel like I do everything right lifestyle wise. Let me know what you all think in regards to trying a very low dose of test to hopefully get me feeling like myself again…

Thank you in advance
