Need advice about unburdening family

Question

I got diabetes two months before I graduated. The only thing I was trained to do, and which made me excited for the future is off-limits to diabetics, so I am more expensive than ever at the same time as I have lost my prospects of meaningful employment.

I am young enough that I probably have decades left to live. My savings are not sufficient to cover even basic treatment for that long. I do not want my wife and parents to be stuck paying for me that whole time. I want to disentangle myself from them financially without hurting their feelings or making them feel unappreciated.

Has anybody successfully had a conversation like that with a loved one? If so, what sort of things did you say/do that made it easier to convince them?
