How Cardiotoxic is NEP esp when combined with other Caths?

Assuming long term use of both NEP and 2mmc, how big is the risk of heart attacks or similar cardiovascular emergencies?

Age 36, no exercise, F, 55kg, history of alcohol and very severe Benzo abuse.

RoA: Intranasal NEP 20mg-40mg 2/3 times during the day. Oral 50mg once. a day.

2mmc basically all day (redosing intranasally every 30 mins) alongside the NEP. Some days I take 200+350mg orally and continue snorting. 1g a day approx

right now I'm kinda keeping a..."balance" by taking benzos and alcohol with the stims to use the depressive effects as a "dampener" on my heart, and Seroquel for sleep.

Taking Caths is...very unpleasant. at first it was insane how good it felt l, and at high doses I can still feel that, but now esp with the NEP my heart feels..."bad". very. but idk if that is just the increased strain from the vasoconstricton
