Breast reduction & lift through insurance

Hi! I’ve had large and saggy breasts (was a 36DD but have lost weight since then and now they’re 36D but so saggy) since I can remember. I didn’t think I’d ever have a chance for a reduction to be covered by insurance until hearing stories lately. Also from entering some measurements online they mentioned i could potentially qualify.

I’m in Nashville TN with BCBS insurance and wanted to get advice on the process for my best chance of insurance coverage? Do i start with PCP/OBGYN or go straight to surgeons? I also have a history of back pain that i have gotten an MRI and PT a few years ago under different insurance. Not sure if this helps? Any advice is welcomed!
