How do i get my cat to stop scratching?

my cat has scratched a portion of her neck raw and bloody. she doesn’t have fleas or ticks, she’s gotten her meds. i gave her a bath with moisturizing pet soap, and the poor girl is still itchy. how do i help her + keep the wound clean?
