Dr K - inconsistencies or vision?

I’m re-watching (binge watching) This Is Us and I’m confused. Dr K dies when young Kate is getting her appendix removed, and his wife was already dead at that point right? But at jacks funeral (kids are 16), he turns up to give Rebecca his condolences- with his wife!! I had to look it up right away - is it a vision? What’s going on? !
