I keep getting my exam accommodations denied and I don’t know what to do

Weeks ago, I put in a request with SLDS for exam accommodations for my Physics 1200 class, and yesterday I get an email citing my exam request has been denied because “Permission to test on requested date/time not obtained” from the instructor. I put in a request to test 4 hours before the standard exam time because the standard exam time slot was full. This is within the 3 day testing window allowed by the course. I email the course director asking if I could arrive early to the exam hall to take the exam and they say no, all exam accommodations must be done at SLDS, and to put in a new request within the 3 day window. I put in another request with SLDS and they deny it because the request was put in late. I do the right thing by booking the exam ahead, and still get denied right before the exam because the course instructor never approved the original accommodation request which allowed within the testing window.
