ACE exam for migraines.

Had an ace exam for migraines on Monday and already received denial letter. The letter says that there is no record of complaints or diagnosis of migraines in my medical records. I know there is. How is that possible? Aren’t ace exams scheduled when there is already sufficient medical evidence? The letter also states that the doctor provided a diagnosis of migraines from the exam. What are my next steps?
