Pregnant and recently suspected for T-cell lymphoma

HSTCL

I am a 36F mother of 1 boy and 10 weeks pregnant to date. Back in 2024, my PCP, during routine annual exam, noticed that my WBC and neutrophils are significantly low despite repeated testing. I feel normal and don’t experience any symptoms/pain. My husband is in the military and we were about to move to a new assignment so I had to wait to get to the new station to get referred to a hematologist. Got to our new station, ran some tests and WBC and neutrophils still low so I finally got referred to a hematologist/oncologist. Hematologist did bone marrow biopsy and flow cytometry and everything came out normal. Diagnosis was idiopathic neutropenia. We were only a year in that area and my husband had orders again to move. So as usual, I tried to establish care with a hematologist/oncologist at our current station. He thought it could be benign ethnic neutropenia but noticed that my spleen is palpable. So I was sent for a scan which confirmed that my spleen is enlarged at 19cm. Hematologist/oncologist didn’t call us or nothing so I assumed it was nothing too concerning. Then come Thanksgiving weekend of 2025, I got so sick for a week with on and off fever. Due to my neutropenia and fear of going sepsis, I went to the ER and they ran the usual tests and scans. Negative for all viral infection, all other organs are normal except my spleen which is now at 22cm. So they sent us out our way, no antibiotics or anything. I eventually got better after a round of antibiotics from the urgent care for what I suspected was a sinus infection to begin with. Now Jan of 2026, I had my follow up appt with my hematologist/oncologist. He saw the ER visit and the spleen finding and decided I need a further work up. So I was scheduled for a PET scan among other things. I took a pregnancy test the day after and by a stroke of luck (or not), it turned positive. So I immediately called my hematologist and they cancelled the PET scan. They however scheduled me for a blood draw for a liquid biopsy. Liquid biopsy result came out to be positive for T-CELL Lymphoma. But because I am pregnant, they can’t do any PET scan to further confirm where it is/where it’s coming from. But I was sent for another full abdominal scan, other organs are normal except spleen that’s now 29cm.

Now I am referred to another hematologist/oncologist who specializes in blood cancer. With the liquid biopsy result and the ultrasound scans of my growing spleen, he is convinced that I have hepatosplenic t-cell lymphoma. How lucky to win the most aggressive and rarest form of cancer while pregnant. Right? Anyway, he is doing a confirmatory work up starting with a bone marrow biopsy which took place yesterday and a possible spleen/liver biopsy that might take place soon after. I am holding on to a slight glimmer of hope that this could be something less aggressive than a hepatosplenic t-cell lymphoma. But all we can do is wait and see. My husband is devastated about the news and the fact that we have a 3 year old is what crushes my heart. The statistics are not so good but the doctor said I am young and healthy so I have a high chance of making it. To be fair, I haven’t felt any symptoms to this date except nausea and fatigue which started when I got pregnant. I am really just at the point of being tired of not knowing what it is. I can’t even be truly happy about this pregnancy that I know might be taken away if I need to go to chemo treatment as soon as the official diagnosis is up. I hate to think about not seeing my child grow up. But it is what it is. I can only hope any pray I can make it through this at some point. Thank you for listening. I just really needed to vent.
