Why can’t I hear/use my phone when I answer it while I drive?

I’m able to use evening else, my phone connects to blue tooth and I can play music on Apple car play. I’m also able to answer when a call comes in. But they can’t hear me once it connects and I can’t hear them.

I have to go to my phone and turn it on iPhone or speaker phone to answer calls or make calls. Do you happen to know what’s going on?
