I built an AI-powered, fully offline triage system for transplant hospitals (and it keeps improving itself)
Hey everyone. I wanted to share a project I’ve been building called TriageLink.
It’s an AI-powered, fully offline triage application designed specifically for transplant hospitals. The goal is simple: reduce cognitive load for transplant triage coordinators while keeping everything 100% local and HIPAA-safe (no cloud dependency at all).
What makes it different:
🔹 Fully Offline AI
Runs locally. No internet required. No cloud storage. No HIPAA exposure.
🔹 AI-Assisted Triage
Analyzes patient complaints and vitals
Classifies urgency
Suggests appropriate paging/routing actions based on hospital-specific rules
🔹 Self-Improving Per Hospital
Each installation continuously refines its reasoning model using locally stored usage data.
It learns the hospital’s specific phrasing, logic, escalation paths, and protocol language over time.
So instead of being a static decision-support tool, it becomes a tailored triage assistant unique to that institution.
Core Modules:
• Real-time Triage Dashboard with AI insights
• AI Protocol Management with rule refinement
• Rules & Paging Management (full CRUD + escalation workflows)
• HIPAA audit logs and compliance reporting
• AI-powered analytics & anomaly detection
• Natural language report generation (“Generate a monthly escalation anomaly report”)
• Role-based access with permission audit logging
• Configurable LLM endpoint (Ollama, OpenAI, etc.) with offline fallback
The system operates with zero cloud dependencies and 100% local persistence.
This is a continuously evolving application — I’m actively refining the AI reasoning engine, optimization workflows, and hospital-specific adaptability. The long-term vision is to create a category of “adaptive clinical AI systems” that improve locally without central data aggregation.
Would love feedback from anyone working in:
• Clinical operations
• Healthcare IT
• AI workflow automation
• Productivity tooling in high-stakes environments
