dumb question

i dont want to mess this up and im getting very confused on what to opt out of in order for meps to not see my records

What I've gathered so far is that atleast 12 entities can see my records

1. Carequality 2. Epic Care Everywhere 3. Athena Health 4. HIE Texas 5. Austin Regional Clinic 6. My Pediatrician 7. The medical group my pediatrician is linked to 8. Ascension Healthcare 9/10. 2 separate entities Ive been to that are owned by Ascension 11. Walgreens 12. Surescripts

Does each one of these places have their own HIE opt out form? I feel like I wouldnt get a response if I sent an email to Carequality asking them to stop giving my records away. And for doctors/hosptials I've seen that are apart of a larger network, do I need an opt out form from that specific doctors office/hospital or a form from their larger network? Also, as far as I can tell HIETexas has no opt out form at all.

If someone could dumb this down for me, I would be very grateful
