Really need advice, bilateral CBS for 8 months now. Offered Therapeutic ultrasound or cubital tunnel release. Have been improving over the past 2 months slowly.

Hi everyone, I’m a 22 yo male that’s had bilateral cubital tunnel syndrome “mild via EMG/NCS in Nov’25” for 8 months or so now from weight lifting. I’m am so lost at this point, I have seen multiple doctors and finally feee like I have a good one. They offered therapeutic ultrasound therapy or releasing the nerves with surgery. I have my left arm scheduled for 3/10 and was very adamant to get it done. But the last 2 months I have finally seen some improvements so now I’m more on the fence about it. Has anyone had any luck with their body healing itself back to normal or had any good luck with therapeutic ultrasound. Over the past few months I’ve had twitching in my left palm near my pinky finger on the side of my hand. I get pain in my thumb and palm area near my thumb. I have started to show radial and carpal tunnel signs when all I’ve done since August is rested, fine motor movements in my are make-it shake now. I can finally play games again and function somewhat normally but have pain and twitching in my arms and if I sleep with my arms bent my fingers and half my palm still go numb. But I have no numbness or tingling at baseline, just pain and pressure. I feel like a lot of my strength is back finally but as I said I haven’t done anything in months so I worry if I go back to an active life I’ll be back to square one. But I also would hate to do surgery and not have needed it.

There’s more to the story but I’ll try to keep this short, I’m not sure what to do at this point any advice is greatly appreciated.
