Person Next to MC Getting Shot

In my story, MC and her friend are helping a woman escape (with the woman being in between the two). Ideally that woman gets shot, but I can't figure out if MC would immediately figure out that the woman was hurt, or if the characters would even be able to tell if the bullet hit any one of them. Is this too unrealistic?
