I'm addicted and hurt and separated from God

I have fallen into addiction. A year ago, actually. Now I'm hurt because I feel like I'm losing heaven. I really am repentful with all my heart and I don't want to be addicted. Can I still make it to heaven even if I can't quit no matter how hard I try? I really want to make it to His Kingdom.
