Long form cdr

I was approved in October 2021 I see my doctor every 3 months I've been on 9 different medications since 2022 I've switched 4 different medications since the end of 2024 I'm currently on 4 I was approved for ssdi for schizoaffective, bipolar, GAD and ADHD .. I follow all treatments and see my doctor 4x a year every 3 months this is my first long form cdr since 2021 I received it this month I sent it back in with a medical record diagnosis and all the different medications I've been on .. I feel like I shouldn't be worried I do everything I'm supposed to do I'm not faking I wish I was tho I go months sometimes with out seeing or talking to anyone due to my paranoia but I can't help but have anxiety do you guys think I should be worried? And have you done a long form cdr and how did it go
