GLP-1 From Birmingham, Alabama

Went thru the MOVE Program and was referred to the endo / pharmacist. Did all the steps and was told this:

Tier 1a: Patients with a life-threatening weight-related condition or

experiencing treatment delay for a life-threatening condition due to

excess weight for whom treatment delay would likely increase mortality

risk within the next year

* Tier 1b: Patients without diabetes who are >/45yo with established CV

disease. CV disease is defined as previous MI, previous CVA or

symptomatic PAD based on inclusion criteria for the SELECT trial.

- From review of chart, patient does not meet Tier 1a or 1b criteria. Consult

not approved and pending orders will be discontinued.

***Consult decisions may be appealed for higher level consideration within 7

days of disapproval by entering a *Non-Formulary Appeal Request/Prior

Authorization Drug Appeal consult.***
