Do I need an optometrist, an opthalmologist, both, a medical doctor or something else?

Hello, I've had terrible eyesight since I was a little kid. I started wearing glasses when I entered kindergarten but likely needed them well before then. My eyesight has progressively gotten worse as I've gotten older to the point that I no longer feel safe driving in dim or dark light conditions and I repeatedly failed the (assisted) vision test last year when I had to renew my driver's license. In order to read anything in print I need contacts, reading glasses, a magnifying glass and a very bright light. I have a lazy eye, bulging eyes, can't see well up close or far away, sleep with my eyes open, dark red blood vessels all through the whites of my eyes, and have terrible floaters. I've been going to an optometrist all my life until recently when I was frustrated that my optometrist never gave me a diagnosis (formal written information about the various eye conditions I have) and never suggested any remedies other than contacts. The doctor was dismissive of my complaints about my worsening vision and feeling unsafe driving in poor lighting. (which has resulted in me being unemployed and unable to find a daytime only job) Two years ago, I finally went to an opthalmologist hoping they would be more helpful. They told me I also have cataracts and are pressuring me to get surgery. However my insurance doesn't fully cover surgery and I can't afford the thousands of dollars out of pocket that the surgery would cost (because I'm unemployed and have no income). Should I bother going back to the opthalmologist knowing I cannot afford the surgery? (and I hate being pressured to do something I can't do, feels like they are just after my money). Or should I just go back to the optometrist to get a new contact lens prescription? or do I need to see both doctors or a different type of doctor? Is there anything else I can do to figure out what exactly is wrong with my vision and/or what I can do (besides surgery) to improve it? All suggestions greatly appreciated, thank you so much!
