Considering Hysterectomy for Adeno & Uterosacral Endometriosis – Advice?

Question

Hi everyone,

I’m looking for some advice and personal experiences because I’m feeling a bit overwhelmed with my options.

I was recently diagnosed with adenomyosis on ultrasound, and I also have pelvic endometriosis in the posterior compartment — retrocervical, both uterosacral ligaments (worse on the right), and pouch of Douglas involvement. My symptoms are bad period pain, bad ovulation pain and painful sex.

Initially, my doctor advised me to continue taking the contraceptive pill, but I explained that hormonal contraception doesn’t work well for me. I experience significant mental health side effects so I’m not keen to continue.

When I asked about surgery, he mentioned presacral neurectomy (PSN) with excision and keeping uterus , but I’m not comfortable with that option.

Since I don’t want children (and my husband is infertile), he then suggested that a hysterectomy (removing the uterus but keeping my ovaries and cervix) along with excision of the endometriosis would likely be the best long-term solution.

If I have a few concerns and would really appreciate hearing from anyone who’s been through something similar:

1.	How likely is it that my ovaries could “shut down” or go into early menopause after hysterectomy if they are left in place?

2.	If ovaries are kept, is weight gain still common?

3.	Has anyone kept their ovaries and still experienced ovulation pain afterward?

4.	If your libido was already low in general before surgery, did it improve after hysterectomy?

I’m just trying to weigh up quality of life vs. risks and long-term effects. Any honest experiences — positive or negative — would really help.

Thank you so much 💛
