Correction on DoA log

2 months After site initiation visit another study coordinator was added to the DoA log, and all tasks assigned fir the first SC were copied for the new one. It was few months ago, and just recently during monitoring visit I noted that new SC does not have access to 2 of the vendor portals which she was assigned to. She didn’t complete any procedures related to this portals, this was done by the first SC who has training and access.

My question is how it would be better to correct the DoA log. Would it be better to just close them with the current tasks and open in the new row with the correct tasks assigned or we can correct with single line strike per GCP and sign with initials on the current row with the current date?
