1x10, 2x09 and 3x20 What's wrong with Foreman?

During 1X10 ( stories ) Foreman acts like an entitled judgmental prick claiming the patient wants to abuse the system to have some free meals and some sleepy time, obviously he's proven wrong in the end, and that patient dies.

Now in the future, 3x20 the same scenario happens again, has Foreman not learned anything from his past experiences?

Again judging before having all the facts, accusing the lady of gaming the system and in the end she dies too.

And small parenthesis, but he acts exactly the same in Deception (2x09), the episode about the hot OTB babe saying she's probably a prostitute and her pimp probably beat her hence her hematomas.
