Chronic paronychia on both my foot

Hello F26, PCOS

I've been having chronic paronychia on both my foot for month now, I've been to the doctor multiple times, I've also seen a professional to cut my nails properly every month and it keeps coming back. There is never pus but the side of both my big toes and my second and fourth toes are swelling after 1 or 2 weeks post cut and it does not stop if I don't put mupirocine on it.

What could it be ? I'm pre diabetic but not diabetic ? It's new I've never had that before, I had a first paronychia in November and it hasn't stopped since.

I'm cleaning my floot everyday and drying them immediately. I have very sweety feet but I don't wear shoes I'm barefoot or in slippers in my house all day. I also have iron deficiency.
