I've tried everything - why won't my mining urns trigger?

As the title says - I can't seem to get my mining urns to trigger.
I'm using decorated and exquisite (both are in my brooch) so whatever I'm mining doesn't matter - but I just did the entire life and death upgrade path without a single urn triggering. I've tried pulling them out of the brooch, I had an urn in my bank at first (1% full) though it used to teleport to me. I brought it along, and no luck. I ended up destroying it to see if it was bugged. Still no luck. No other, older urns in my bank that are awaiting teleportation. And yes - these urns are empty (rune added).

Anything else I might be missing?? I just came back to the game a month ago and it's been driving me insane.
