Free 120 question (SPOILERS DO NOT LOOK IF YOU DO NOT WANNA SEE)
hello!
I was wondering if anyone could clarify the findings of this peripheral blood smear from question 25 (block 2) in the Free 120
The question stem very obviously clues towards a folate deficiency given the heavy alcohol use, poor diet and anemia, BUT, the peripheral blood smear threw me off.
It has the classic hypersegmented neutrophil of folate deficiency, but to me it also looked like there were target cells, Howell-jolly bodies and bite cells which really threw me off and made me think about G6PD deficiency.
Can someone help me concretely understand why folate and not anything else given blood smear? maybe I am simply misinterpreting the blood smear, lmk!!
Edit: it would not let me add the photo so I apologize, but here is a link to the pdf (page 31, question 65 overall):
