SBR AP5 LT/T94

Seeing conflicting info about the model for the LT/T94 to go on the 5320.23 / Form 1. I filed today with just T94 as the model. I included a photo of the serial, MKE/Century marking, and then the T94 stamp on the barrel. VA will be signing an AWB here soon and I will have till July 1 to get approved so I likely need this done correctly the first time.

Does this sound correct?
