I mess up

Workplace Challenges and Conflicts

Hi everyone. I’m really anxious and could use some perspective from other clinicians.

I’m a provisionally licensed counselor working as a 1099 contractor. I recently realized I made a documentation mistake in a note that I’m now terrified could get me in serious trouble.

In the note, I documented that I spoke with a client’s mother about the client’s behavior and that I worked with the client during the session. The problem is that the mother was actually hospitalized at the time the note was submitted (I later learned she was on a ventilator). So the documentation about speaking with her is inaccurate.

To be clear: I did see and work with the client, and my clients have confirmed to the organization that I showed up and provided services. But the part about the parent contact is wrong and I’m really upset with myself for submitting it that way.

Adding to my anxiety, some of my clients told me the organization has been calling them asking whether I came out for services. I have a meeting with my supervisor tomorrow and now I’m spiraling thinking they’re building a case against me, that my contract will be terminated, or even that my license could be reported.

I know documentation integrity is extremely important, and I’m prepared to take responsibility and correct the note if possible. I’m just trying to understand realistically:

How serious is a situation like this typically viewed in practice?

What’s the best way to address this in a meeting with a supervisor?

Has anyone dealt with correcting documentation after realizing an error like this?

I’m really beating myself up and trying to handle this professionally, so any perspective would be appreciated.
