Bergara barrel came loose

Hand tightened a deadair keymo mount to my hmr put on my sandman S not even tightened all the way down. Went to remove my sandman with minimal to no force and my entire barrel came loose. I'm really upset with how under torqued this barrel was for a 1k gun.

1. Has this happened to anyone?

2.D id you send back to bergara or have a gunsmith snug it down.
