Flare Up treatment

I had a micro at l5 s1 in 2013. Repeat MRI a month out showed reherniation. I fought and did 2 years PT, another ESI and massage to break up the scar tissue. Now I’m finally worried I’m in store for either more ESI or surgery. I just took a steroid pack. I was better about 4 days now flaring again. I have new insurance and my old pain mgmt doc is no longer there. What’s my course of treatment? Do I have to go back to PCP, then do PT, MRI, then hopefully pain mgmt for a shot, or do I call my surgeon straight? He said he knows I’ll be back someday.
