What fetishes will you add?

For example, I'd like for some characters to be able to circumcise Hiro, leaving the head of his penis permanently inflamed and sensitive to touch. This procedure could be performed by a nurse at the sperm bank for a medical examination, by Urka during the protagonist's enslavement, or by the brothel owner. At the same time, Hiro, who had undergone this surgery, would not have been able to become impotent - his penis would always appear as half-erect from now on.
