Long-term problems with vulva

NSFWPhysician Responded

English is not my primary language and therefore some words may be grammatically wrong!

I have had problems on and off for about two years (a little bit more) and I am a female, 37 years old, with two children who I gave birth to vaginally without complications 10 and 8 years ago. I am at average height and weight.

I do also have well treated multiple sclerosis, stabilised and not experiencing any symptoms from that disease. (diagnosed at 22). I have been on rituximab for 13 years.

I did have a gastric bypass when I was in my 20s.

I am currently on birth control (Slinda or Slynd) and have been using that contraceptive for almost three years.

My issues started with being more dry even though I was aroused. Then all of the sudden after sex I got several cracks in the skin (like razor cuts).

after that it has become more severe with a burning sensation in the vulva, and especially above the clitoris and at my perineum. From time to time the cuts come back and i do call this an "outbreak". The burning is consistent, but the cuts are around every two months. Sometimes it also looks like the skin is "burned away" in small areas.

I don't have any weird discharge, I have a lot less discharge overall due to being so dry vaginally. The smell is normal and my pH is normal (due to testing).

My doctor has been stuck with "this must be Candida" and have tested me numerous times and sometimes yes, it has been a fungal infection, but more often not.

I have also had a lot of urinary tract infections and I personally feel that this also is an effect from being dry.

At one time, another doctor mentioned lichen sclerosis and I'm currently waiting to be tested for that. I did at that time have some small white patches, that are gone now.

But I think this may be something else because one time when I had my latest UTI, I also had an "outbreak" at the same time (with cuts and burns) and the antibiotics prescribed for the UTI that time actually cleared my outbreak within a few days! (an outbreak often lasts from 2-3 weeks and are very limiting for me, even walking is problematic because of the burning)

I don't think this is an STD, I've been tested before but maybe not for the more "rare" STDs. So who knows?

I am in a long-term relationship and still have sex, but not during bad days.

TL;DR Female with burning sensation in vulva for long time, wich you can physically see.

The intensity varies but are constant.
