TSH increased after increase in levothyroxine?

Labs/Advice

Hey all, I don’t usually go to reddit for medical advice, but this time is different. I was diagnosed with Hypothyroidism 3 years ago (although I’m pretty sure I’ve had it for much longer), and I’ve been on 25mcg of levothyroxine since then.

Recently I’ve been feeling really sick: extreme fatigue, muscle aches, muscle weakness, chills, feeling cold literally all of the time, weight gain, and depression (to the point where I have passive suicidal thoughts and thoughts of self harm). I went to my PCP because I don’t have an endocrinologist at the moment, and she did some bloodwork.

TSH: 3.04 mlU/L

T4 free: 1.2 ng/dL

T3 uptake: 25%

I have an extensive family history of hypothyroidism, and everyone in my family typically feels better on the lower end of “normal” (my grandmother feels at her best at 1 mlU/L), so I told my PCP this. She increased my levothyroxine to 50mcg. I started feeling better, at least for a couple of weeks, and then started to feel awful again. I had updated bloodwork last week after 6 weeks of being on the increased dose of levothyroxine, and these were my results:

TSH: 3.52 mlU/L

T4 free: 1.3 ng/dL

T3 uptake: 30%

I had my follow up appointment today, and my PCP is putting me back on 25mcg levothyroxine. I don’t think this is the right course of action, but I’m being denied a second opinion because “every other physician will agree with her decision”. My question is, has anyone else ever experienced something like this where your TSH increased after a levothyroxine increase? I can’t find any information online about this happening. Is that something I should be concerned about? And should I decrease my dose of levothyroxine at the risk of becoming really sick again?

I feel so invalidated right now…
