Complete SSA-3 online

I’m applying for spousal benefits.An SSA-3 was requested by email.

I called my local office with questions.

The representative said she would email me form SSA-3.

The email she sent looks like it’s to be completed online.

Is this correct? I know it needs my husbands signature.

She also requested I bring our marriage certificate into the office.
