Godparent Affidavit

Hello! Thanks for reading in advance Father.

I am scheduled to becoming a Godparent April 17th. I am attempting to fill out the Godparent affidavit however I have not consistently been going to my parish.

I have been baptized, had first communion, been confirmed and also attended Catholic school from 1st to 8th grade. Is there a way to get approval from my local parish? I am in the Bay area in CA and my god son's parish is in Nevada.
