how to make my property and finances safe for my daughter and not the state

i have recently had a heart attack i am 55 this year and i want to make all my finances safe just incase anything happens to me or my wife of 32 years i dont want to pay a penny to the state for myself or my wife to be looked after in a home we have contributed to the state our entire adult life i want my 29 year old daughter to be looked after what should i do
