CSC FORM 211 MEDICAL CERTIFICATE

Hello! Just had my CSC Form 211 filled out sa RHU dito sa amin after undergong the required tests. And signature, license number, and designation lang yung nilagyan nung doktor, leaving other information blank like the height, weight, and blood type. Okay lang po ba yun? I'm passing the documents needed as a COS worker. Please someone answer.
