What's the state of the game/end game?

I haven't been on for a couple of months. I know they added an Invincible boss, something called True Vault Hunter Mode/UVHM6(?), and just recently a small DLC. Last time I played, going up in UVHM didn't have any benefit. So I stayed at UVHM1 to farm gear and complete things after the campaign. Has anything been changed to UVHM levels to have benefits for the player? Or are the drops etc still the same across the UVHM levels?

I was going to wait for the first main DLC and C4SH to release to jump back in. But I've been feeling the itch to play.
