The Story I Want For Fallout 5
I will just get to the point instead of prefacing: The vault you start with is a cloning vault. The first clone went out into the wasteland and lost the GECK that was stored in the Vault. You and your predecessors have been created to go out and find the GECK and use it to fix the wastes. Throughout the wasteland will be bodies that look just like however you designed your character. The bodies will have clues to tracking down the previous dead clone providing more details based on how far they got along in your quest and what interaction they may have had with various factions and side characters that will hint at why they treat you a certain way.
Everyone you run into knows you are a clone and treats you as such, but they don't know whether you are one they have already met before or a new one. Characters will act like you owe them money, and you will either be a scapegoat or conned throughout the game.
There will be a one save mode, where every time you die, you are immediately reloaded back at the Vault. Your level is unchanged, all your equipment is on the dead body of the clone that just died in the place it just died. All your quests are kept because everyone thinks you are still the same clone.
This is just the base premise of the game, the story of what killed the first clone, what happened to the GECK, factions and the clones' interactions with them, side characters, setting, etc. could be designed in many ways.
