Tips to not be scared to go back to the doctor?

So I have been on T for several months now, the agreement with my doctor was to come in and do lab work every 3 months to make sure dosage was right and everything is okay. I came in month 3 as soon as I could despite my terrible fear of needles but when I had my blood taken I passed out and convulsed apparently. I'm fine, doctor explained it's something that happens. I said I was alright it was just a little weird. But I have been dreading to go back in again. It's been past month 6 but I just keep making excuses like well money has been tight the extra lab fee is too much right now (Not entirely untrue) I'll just hold off a little it should be fine. But when I put in my refill request just a couple days ago I got "On hold until can get approval from prescriber" even though I have refills remaining. I'm afraid I've been cut off until I come back in again maybe? I know I need to, is there any advice on what I can do to avoid passing out?
