I'm confused on mod plans, I never seem to get any.

After playing since XMAS and scrapping hundreds of modded items I have only unlocked a handful that I can craft, and even very few actual mods themselves. Can someone explain to me how this system works? I know the % chance is very low, but I feel like I am doing something wrong.
