At my wits end with my patellar instability, PT seemed to be helping but recently it’s been acting up more

Need Help

I’m 32/F, in my early twenties I experienced two patellar subluxations, once on each knee, a couple years apart. The first time I had no idea what happened other than a sudden warmth and the worst pain I’ve ever felt, I saw white and immediately collapsed to the ground on solid concrete. The second time I was walking along, felt the same blinding pain, looked down just in time to see my kneecap popping back into place.

Saw a Dr at the time who took x rays and diagnosed it as patellar instability, sent me to PT and it seemed to improve after that once I developed a modest strength training routine.

Began feeling unstable again occasionally last year, went to a new PT since I live in a different area now, got diagnosed with HSD (nearly meet the criteria for hEDS, but not quite). The news was tough, but not surprising. Still, I thought I was still making progress! But recently, my right knee has begun to act up again despite sticking to my exercises. Occasionally buckling, and feeling like it’s trying to tweak out of place mid-walk. I walk long distances frequently because I live in a city and don’t need to drive, so this has been really difficult and made me nervous to leave the house, since when it happens in the middle of a walk I have to basically limp home. As a women who is not at all physically imposing it makes me feel like a walking target.

My PT has been oddly dismissive of this and wants me to focus on my exercises despite my concerns. She discouraged me from buying a stabilizing brace because obviously you aren’t working your muscles by using one, I get that, but the fear of a subluxation is kinda taking over my daily routine. I want to get a second opinion, is there another type of specialist I could see to determine if something else is going on? I haven’t seen any other doctors since my HSD diagnosis and I’ve felt really overwhelmed and confused.
