TA-Therapeutic Alliance scores & convos

I’m on a platform that requires clients to do

a PHQ-9, GAD-7 and TA (therapeutic alliance assessment). A couple longer term clients gave me scores above 15 (15-20 high TA), however, recently I noticed the score for “cultural competency” went down a point for a couple clients. I am the same ethnicity, gender and sexual orientation of both. What are some constructive ways I could bring this up in a session? How to inquire curiously and non-confrontational? Yes I have a tiny bit of anxiety about how to breech subject. Thanks for any feedback.
