Dog in Severe Pain after Physical Exertion

Hello, my 7 y/o male chocolate lab (not sure if purebred he’s a rescue) engages in screaming fits and severe pain anytime he gets the zoomies for too long or is active for too long. We’ve asked many vets and they have no idea. Has any one else experienced this?

Dog will get zoomies, then do circles, downward dog while screaming in pain for a couple of minutes and then go back to normal.
